Overview

Prosperemos Juntos / Thriving Together currently engages coalitions in Bexar County, the Mid-Border region (Dimmit, Edwards, Kinney, LaSalle, Maverick, Real, Uvalde, Val Verde, and Zavala counties), and the Laredo area (Webb, Zapata, and Jim Hogg counties). For the 2024 cohort, we will target our Mid-Border and Laredo regions. Our target counties include Jim Hogg, Zapata, Webb, Dimmit, Edwards, Kinney, LaSalle, Maverick, Real, Uvalde, Val Verde, Zavala. Each coalition will be supported as they:

  1. Engage in community transformation in a way that advances equitable processes and outcomes.
  2. Address the underlying vital conditions (social determinants of health) with communities most affected by poverty, racial, health and other inequities.
  3. Develop and implement a strategy to address root causes of intergenerational poverty and exclusion.

Prosperemos Juntos | Thriving Together is a power-shifting, collaborative opportunity for communities in south and central Texas to substantially accelerate their equity journey. We believe communities hold the solutions to improve their health and well-being. The goal of the Learning Collaborative is to equip coalitions with skills and resources to strategically advance health equity, using the Pathways to Population Health Equity framework as a guide.

As part of Prosperemos Juntos | Thriving Together, we hold intentional space for faith communities from all religions and denominations called Community Faith Connections. We host a special monthly session in which we explore the role faith communities play in neighborhood change. In these sessions, we also offer tools and resources that uniquely apply to faith contexts. Faith communities bring unique capacities and are instrumental contributors to cultivating a thriving community. All faith communities who are walking with communities experiencing inequities are strongly encouraged to apply.

The Two Phases

Phase I

Phase I, the Learning Collaborative, lasts six months (from March – August). During this phase, Methodist Healthcare Ministries shares various frameworks and tools with coalitions to equip them to develop and implement a health equity strategy. The curriculum for the Learning Collaborative was developed in partnership with WE In the World and Rio Grande Valley Language Justice Collective.

Phase II

After completing the Learning Collaborative, coalitions are invited to apply to Phase II, the Implementation Phase. The Implementation Phase lasts 3 years and includes funding as well as capacity-building resources. While coalitions focus their time working in community, they will also have opportunities to learn from and collaborate with each other through convenings and other events.

Ideal Applicant

For this invitation, we are looking for coalitions with a shared or common agenda for improving health and wellness. An ideal applicant is a coalition that centers persons with recent lived experience of inequity and has at least three organizations. The ideal coalition will:

  • Be located in one of the following counties: Dimmit, Edwards, Jim Hogg, Kinney, LaSalle, Maverick, Real, Uvalde, Val Verde, Webb, Zapata, or Zavala counties. For Bexar county, we will only accept applications for coalitions working in the Eastside neighborhood.
  • Advance health, well-being and equity
  • Apply a racial equity lens
  • Work across organizational boundaries
  • Set goals for improvement of how the community/coalition functions
  • Develop a strategic plan to support the resiliency of families and build a thriving community
  • Be experienced or excited to learn how to work collaboratively as a coalition
  • Be devoted to meaningful engagement of persons with current lived experience of inequity 
  • Be motivated to advance approaches that address the root causes of community problems
  • Use data to inform decision-making and share data as appropriate with partners
  • Be part of an effective and transformational coaching and peer-mentoring process
  • Adapt and implement best practices that are relevant to its community
  • Be ok with making mistakes and being honest about their learning journey
  • Move beyond transactional relationships and practice deep listening

The Learning Collaborative

During the first phase of this effort, we will help coalitions build your team, partnerships, and strategy. We are not looking for “perfect coalitions.” This Learning Collaborative is designed to help supplement your community’s work – not replace it nor “reinvent the wheel”. We are looking for groups that are hungry to be effective and strategic in giving everyone a fair chance to have good health and a good life.

We will accept a maximum of 15 coalitions per cohort.

The ability to meet, connect, and build with different community members and organizations across South Texas. I am inspired and energized when I get to connect with other folks that are connected to the lands and communities of Tejas.

I am a baker so it is hard to choose when I have so many favorites. A decadent cake, a warm cookie, a rich cheesecake, tiramisu, or pan dulce depending on the day.

Jaime Wesoloski

President & Chief Executive Officer

Jaime Wesolowski is the President and Chief Executive Officer at Methodist Healthcare Ministries. A healthcare executive with three decades of leadership experience, Jaime is responsible for the overall governance and direction of Methodist Healthcare Ministries. Jaime earned his Master’s Degree in Healthcare Administration from Xavier University, and his Bachelor’s of Science from Indiana University in Healthcare Administration. As a cancer survivor, Jaime is a staunch supporter of the American Cancer Society. He serves as Chair of the American Cancer Society’s South Texas Area board of directors and he was appointed as Chair to the recently created South Region Advisory Cabinet, covering eight states from Arizona through Alabama. Jaime believes his personal experience as a cancer survivor has given him more defined insight and compassion to the physical, emotional, and spiritual needs of patients and their families.
