Overview

Prosperemos Juntos / Thriving Together currently engages coalitions in Bexar County, the Mid-Border, and the Tri-County.. For the 2026 cohort, we will target the Rio Grande Valley region. Each coalition will be supported as they: 

  1. Engage in community transformation in a way that advances equitable processes and outcomes.
  2. Address the underlying vital conditions (social determinants of health) with communities most affected by poverty, racial, health and other inequities.
  3. Develop and implement a health equity strategy to address root causes of intergenerational poverty and exclusion.

Prosperemos Juntos | Thriving Together is a power-shifting, collaborative opportunity for communities in south and central Texas to substantially accelerate their equity journey. We believe communities hold the solutions to improve their health and well-being. The goal of the Learning Collaborative is to equip coalitions with skills and resources to strategically advance health equity, using the Pathways to Population Health Equity framework as a guide.

As part of Prosperemos Juntos | Thriving Together, we hold intentional space for faith communities from all religions and denominations called Community Faith Connections. All faith communities who are walking with communities experiencing inequities are strongly encouraged to apply.

Cross lines

The Two Phases

Phase I

Phase I, the Learning Collaborative, lasts six months (from March – August). During this phase, Prosperemos Juntos | Thriving Together team shares various frameworks and tools with coalitions to support them in developing and implementing a health equity strategy.

Phase II

Phase II, after completing the Learning Collaborative, coalitions are invited to apply to Phase II, the Implementation Phase. The Implementation Phase lasts 3 years and includes funding as well as capacity-building resources. While coalitions focus their time working in community, they will also have opportunities to learn from and collaborate with other coalitions through convenings and other events. During this phase, coalitions are encouraged to continually recruit diverse members — particularly those with lived experience — and to adopt intentional strategies that shift power and decision-making to community members most impacted by inequities.

The Ideal Applicant 

For the 2026 Learning Collaborative cohort, we are looking for coalitions with a shared or common agenda for improving health and wellbeing. An ideal applicant is a coalition that centers persons with recent lived experience of inequity and is made up of at least three organizations. The ideal coalition will:

  • Create clear goals to help the community /coalition work better together
  • Be experienced or excited to learn how to work collaboratively as a coalition
  • Be devoted to meaningful engagement of persons with current lived experience of inequity
  • Be motivated to advance approaches that address the root causes of community problems
  • Be okay with making mistakes and bein honest about their learning journey 
  • Move beyond transactional relationships and practice deep listening
  • Continuously brings I new members with lived experience to strengthen the coalition
  • Be located in the Rio Grande Valley 
  • Advance health, well-being and equity
  • Think about how different racial groups might be affected and aim for equity
  •  Work across organizational boundaries
  • Collaborate with organizational partners and community members
  • Create clear goals to help the community/coalition work better together 

The Learning Collaborative

During the first phase of this effort, we will help coalitions build your team, partnerships, and strategy. We are not looking for “perfect coalitions.” This Learning Collaborative is designed to help supplement your community’s work– not replace it nor “reinvent the wheel”. We are looking for groups that are hungry to be effective and strategic in giving everyone a fair chance to have good health and a good life.

We will accept a maximum of 8 coalitions per cohort.

You don’t need to be “perfect,” but you do need to be ready to roll up your sleeves with your peers across this region and get to work. We are eager to support you in this journey, work with you to help create meaningful connections, and help ensure that your community health improvement efforts are successful!

The Coalitions Experience

To us, ‘thriving together’ means building a community where everyone feels seen, valued, and connected to a shared purpose. It’s about creating spaces where we can question systems, embrace change, and empower one another to lead with courage and compassion. Jaime Wesoloski

President & Chief Executive Officer

Jaime Wesolowski is the President and Chief Executive Officer at Methodist Healthcare Ministries. A healthcare executive with three decades of leadership experience, Jaime is responsible for the overall governance and direction of Methodist Healthcare Ministries. Jaime earned his Master’s Degree in Healthcare Administration from Xavier University, and his Bachelor’s of Science from Indiana University in Healthcare Administration. As a cancer survivor, Jaime is a staunch supporter of the American Cancer Society. He serves as Chair of the American Cancer Society’s South Texas Area board of directors and he was appointed as Chair to the recently created South Region Advisory Cabinet, covering eight states from Arizona through Alabama. Jaime believes his personal experience as a cancer survivor has given him more defined insight and compassion to the physical, emotional, and spiritual needs of patients and their families.
