Methodist Healthcare Ministries Renews NCQA Patient-Centered Medical Home Recognition
San Antonio, TX (October 1, 2026) – Methodist Healthcare Ministries of South Texas, Inc. (MHM) is proud to announce the successful renewal of its National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home recognition, reaffirming the organization’s commitment to delivering high-quality, coordinated, whole-person care that places patients at the center of every decision. The NCQA Patient-Centered Medical Home program recognizes healthcare organizations that demonstrate a commitment to patient-centered care and continuous quality improvement.
Through an integrated approach that combines medical, behavioral, and oral healthcare with health education and support services, MHM works to address each patient’s unique needs while improving health outcomes and enhancing the care experience.
“Renewing our NCQA Patient-Centered Medical Home recognition reflects our unwavering commitment to providing compassionate, high-quality care that is centered around each patient,” said Jaime Wesolowski, President and CEO of Methodist Healthcare Ministries. “Every day, our teams work to ensure patients receive the right care at the right time while addressing the full spectrum of factors that influence health and well-being. This recognition validates that commitment and underscores our dedication to continuous improvement.”
The NCQA Patient-Centered Medical Home program was developed with input from organizations including the American College of Physicians, American Academy of Family Physicians, and American Osteopathic Association. The standards emphasize systematic, patient-centered, coordinated care that supports access, communication, and patient involvement.
The recertification recognizes MHM’s ongoing efforts to strengthen care coordination, enhance patient access to services, leverage data, and quality improvement practices, and foster strong relationships between patients and care teams. By continuously evaluating and improving clinical operations, MHM strives to ensure every patient receives personalized care that supports their physical, emotional, and behavioral health needs.
“This achievement is a testament to the dedication, skill, and compassion of our clinical staff and support teams,” Wesolowski added. “Their commitment to excellence makes it possible for us to fulfill our mission while building healthier individuals, families, and communities throughout South Texas.”
Patient-Centered Medical Home Recognition is considered one of the healthcare industry’s leading models for delivering coordinated, patient-focused care. Through this renewed recognition, MHM continues to demonstrate that it has the systems, processes, and resources necessary to provide high-quality care while improving patient experiences and outcomes.
