SUMMARY OF BENEFITS

Health Insurance
MHM pays 100% of team member’s premium and 65% of dependent coverage. Enrollment effective first day of the month following 60 days of employment.

Dental Insurance
MHM pays 100% of team member’s premium. Enrollment effective first day of the month following 60 days of employment.

Retirement Plan (403b)
MHM will match 100% of team member’s contribution up to 7% of salary after one year of employment.

Flexible Benefits (Cafeteria) Plan
‘Pre-tax’ benefits to team members for dependent care and unreimbursed medical care.

Vision Insurance
MHM contributes to team member’s and dependent’s premium. Enrollment effective first day of the month following 60 days of employment.

Life Insurance
MHM pays 100% of team member’s premium.

Short & Long Term Disability
MHM pays 100% of team member’s premiums.

Wellness Benefit
Reimbursement of up to 60% of cost (up to $500 per year) for allowable expenses such as health club membership, nutrition programs and smoking cessation programs.

Vacation
Two weeks per year for the first 5 years of employment, and is available to all team members after six months of employment; accrual increases with tenure.

Sick Leave
Twelve days per year. Sick leave may be taken upon accrual.
