SUMMARY OF BENEFITS

Note: All Benefits are subject to change the following benefits are provided for employees who work at least 32 hours a week:

HEALTH INSURANCE DENTAL INSURANCE
MHM pays 100% of employee’s premium
MHM pays 36% of family coverage
Enrollment effective first day of the month
following 2 months of employment.
MHM pays 100% of employee’s premium
Family coverage available at employee’s expense

Enrollment effective first day of the month
following 2 months of employment.

VISION INSURANCE RETIREMENT PLAN 403B
All coverage is at employee’s expense.

Enrollment effective first day of the month
 following 2 months of employment.

(PLEASE NOTE: employee’s premium deductions for
Medical, Dental, and Vision Insurance begin after
30 days of employment.)

Employee may begin contributing upon employment. Employee match begins on January 1 or July 1 after completion of one year of employment.
MHM will match 100% of employee contribution up to 7% of salary, subject to IRS limits.
FLEXIBLE BENEFITS (CAFETERIA) PLAN SHORT & LONG TERM DISABILITY

Plan provides “pre-tax” benefits to employees for dependent care and unreimbursed medical care. Annual enrollment.

Covers 60% of salary through age 65.
MHM pays 100% of premium. Short Term Disability has a waiting period of 29 days (eligible period).
LIFE INSURANCE VOLUNTARY BENEFITS

MHM pays 100% of premium at a rate of 1.5 time’s annual salary.

Optional at employee’s expense. Includes benefits such as cancer, heart and stroke, and accident policy. Some benefits may be “pre-tax” benefits. Annual enrollment in January.
WELLNESS BENEFIT PROGRAM SICK LEAVE
ELIGIBILITY: In keeping with the goal of promoting wellness for the employee population of MHM, regular employees who have completed at least six (6) months of continuous full time service are eligible to receive this benefit. The benefit is for MHM employees only
ALLOWABLE EXPENSES: Expenses will be reimbursed at 60% of the cost – up to $500 per employee.
A Health Club with certified professional trainers
B Nutrition Programs that are generally accepted and recognized as valid and/or certified; e.g., nutrition classes and disease related dietary programs, etc.
C Smoking Cessation Programs and/or products.
Accrues at 3.7 hours per pay period, 8 hours per month. Sick leave may be taken upon accrual.
A Wellness Benefit is available when an employee’s sick balance accrual goes beyond a 360 hour maximum. Employees are paid for sick leave at a rate of one hour of pay for every two hours of unused sick leave. Calculation is based on hour accrued as the last pay period of each calendar year, with the employee receiving payment in January.
VACATION  

Accrues at 3.08 per pay period for the first five years of employment. Vacation leave is available after 6 months of employment.
